Predesignation of Personal Physician – LAA (LAX, SFO)
California-Based Flight Attendants: In August 2014, employees based in California should have received a letter from Harbor Health Systems explaining the recent change to the Pre-designation law. Now all California-Based employees are required to update their pre-designated physician information. The pre-designated physician change was included in an APFA Hotline message dated 9/13/14.
It has recently been brought to our attention that some Flight Attendants based in California have been notified stating their pre-designated physician form is no longer valid. If an employee does not have a pre-designated physician on file in the event of an Injury on Duty (IOD) claim, they will be required to select a physician from the Worker’s Compensation Network. In California, it is the Medical Provider Network (MPN), 855-854-6141 or www.harborsys.com/genexmpn. If you wish to have your own physician on file in the event of a Worker’s Compensation claim (IOD), you may complete the following form: Predesignation of Personal Physician. The form must be completed prior to filing a new IOD claim.
The completed form needs to be sent to:
Harbor Health Systems
P.O. Box 54770
Irvine, CA 92619-4770
Be sure to provide a copy of the form to your FSM to be included in your P-file (personal file). In order to be eligible for Salary Pay Continuance under Section 27 of our JCBA, the pre-designated physician must be in the MPN. Any questions regarding the pre-designated physician requirements may be sent directly to APFA National IOD Chair, Bellia Peckson at [email protected].
