COVID-19 Vaccine Submission and Confirmation Process
The COVID-19 vaccine is now required to fly to several destinations. As of today, the list includes all destinations in Canada, Colombia, Suriname, and Guyana, as well as Rio de Janeiro and Lima. Munich requires either vaccination or a negative COVID test prior to departure. This list will likely grow. In many cases, these requirements have gone into effect with little to no advance notice.
Flight Attendants without proof of vaccination on file when these requirements go into effect risk being removed from sequences. The Company has not agreed to pay protections for these removals.
If you are vaccinated, but have not yet submitted proof of vaccination, please do so via the form in Comply365 > My Forms > All Forms > AAFA > COVID 19 Vaccination Verification FLT SVC.
To access Comply365 on your Company tablet:
Navigate to My Forms > AAFA > and select the COVID 19 Vaccination Verification form.
To access Comply365 on a personal computer or mobile device:
Complete the form and upload an image of your vaccine card, making sure your name and vaccination date(s) are visible. If you need more than one dose, complete the form only after your second dose. Make sure you tap the blue Upload icon after you have selected your image.
Once you have submitted vaccination verification and it is reviewed and approved by the company, your training record will be updated with SPL 21 code on your HI*9 in FOS. We encourage all Flight Attendants who have submitted proof of vaccination to verify the code has been entered. You can access your training record by inputting the HI*9 code in your personal mode in DECS. It will appear at the bottom of the screen.
You can also access your HI*9 in mobileCCI.
